The Role of Cushions and Backs in Skin Protection - Part 4

If you’re caring for someone who spends most of their day in a wheelchair, one of the most important things to get right is their seat cushion and back support.
Not all cushions are the same. Some are made for comfort. Others are designed to protect the skin, support the body, and reduce pressure. Choosing the right one can prevent serious pressure injuries—and even save lives.
What Makes a Good Cushion?
The best cushions for skin protection are made from materials that spread out pressure and allow the skin to breathe. These might include:
- Gel cushions – Keep skin cool and reduce shear
- Air cushions – Adjust to the body and move with it
- Foam cushions – Easy to use and offer stable support
- Hybrid cushions – Combine foam and gel or air for extra protection

Why Back Supports Matter Too
The back of the chair does more than keep someone upright. It helps with posture, breathing, digestion, and comfort. A poor back support can lead to leaning, sliding, or skin breakdown on the spine and shoulders.
Custom back supports can:
- Improve sitting balance
- Reduce the risk of scoliosis and leaning
- Make it easier to use a tilt or recline feature.
Medicare and Complex Rehab Seating
Medicare may cover a Group 3 Complex Power Wheelchair with tilt and/or recline if the person:
- Has a neurological or neuromuscular condition (like stroke with hemiplegia, Parkinson's, Spinal Cord Injury, ALS, MS)
- Is non-ambulatory and cannot shift weight independently
- Has a history or high risk of pressure injuries
- Is evaluated by a physical or occupational therapist and an ATP
- Unable to perform independent pressure reliefs during wheelchair use.
