Effective October 28, 2026, CMS adds certain manual wheelchair codes to its Required Face-to-Face Encounter and Written Order Prior to Delivery List. This Education post is a documentation and workflow awareness alert for therapists, physicians, and care-team partners—not a coverage guide. On July 30, 2026, CMS published a Federal Register notice that updates the Master List and moves selected DMEPOS items onto the Required Face-to-Face Encounter and Written Order Prior to Delivery List. Among those additions are several manual wheelchair HCPCS codes. The changes take effect October 28, 2026.

Medical Equipment Specialists (MES) in Stuart serves therapists, physicians, discharge planners, and families across Martin, St. Lucie, and Palm Beach counties. We are sharing this so referral partners can plan documentation workflows with official CMS sources—not competitor summaries.

Scope note: This article explains what is changing on CMS’s documentation lists. It does not explain who qualifies for Medicare coverage, how to get coverage, or insurance options.

What’s Changing

For items on the Required Face-to-Face Encounter and Written Order Prior to Delivery List, CMS requires:

  • A face-to-face encounter with the treating practitioner within six months preceding the order.
  • A complete written order prior to delivery (WOPD) for items on that list—before the item is delivered.

CMS also publishes a separate Required Prior Authorization List. Selection for face-to-face / written-order requirements is not the same as selection for prior authorization. Always check the official CMS lists for the code in front of you.

As of October 28, 2026, CMS’s Required Face-to-Face Encounter and Written Order Prior to Delivery List (PDF) includes 105 items.

Which Manual Wheelchairs Are on the List

Per the CMS Required Face-to-Face Encounter and Written Order Prior to Delivery List (PDF) (effective date 10/28/2026), these manual wheelchair codes are included:

  • K0002 — Standard hemi (low seat) wheelchair
  • K0003 — Lightweight wheelchair
  • K0004 — High strength, lightweight wheelchair
  • K0005 — Ultralightweight wheelchair
  • K0006 — Heavy duty wheelchair
  • K0007 — Extra heavy-duty wheelchair

The July 30, 2026 Federal Register notice also added E1161 (manual adult size wheelchair, includes tilt in space) among other non-wheelchair codes. This Education post focuses on the K0002–K0007 manual wheelchair bases that referral partners ask about most often.

Prior authorization note (K0005 only): CMS also selected K0005 (ultralightweight wheelchair) for the Required Prior Authorization List, with prior authorization as a condition of payment beginning nationwide on October 28, 2026. That is separate from the face-to-face / written-order requirement. Confirm the current list on CMS’s DMEPOS prior authorization pages before you submit or coordinate an order.

What Therapists & Physicians Should Know

When a listed manual wheelchair is part of the care plan, timing and documentation matter:

  • Plan the treating practitioner’s face-to-face encounter so it falls within six months before the written order date.
  • Document the encounter in the medical record with beneficiary-specific subjective and objective information tied to the clinical condition for which the equipment is ordered.
  • Send a complete written order to the supplier before delivery for items on the Required Face-to-Face / WOPD List.
  • Standard written-order elements CMS lists include: beneficiary name or MBI, item description, quantity if applicable, treating practitioner name or NPI, order date, and treating practitioner signature.
  • If the encounter is via telehealth, CMS notes that telehealth service and payment requirements must be met.

MES coordinates with therapists, doctors, and care teams. Call us early when a listed code may be involved so paperwork and delivery sequencing stay aligned.

What Providers & Clients Should Expect

For suppliers and for clients working with a care team, expect a clearer “papers first” sequence for listed manual wheelchairs:

  • The face-to-face encounter and complete written order need to be in place before delivery when the HCPCS code is on the Required Face-to-Face / WOPD List.
  • Suppliers must maintain the written order/prescription and supporting documentation from the treating practitioner and make them available to CMS or its agents upon request.
  • For K0005, expect the additional prior-authorization workflow CMS has announced for that code—again, as a documentation condition of payment, not as advice about coverage eligibility.
